Histopathologic Study of Secondary Syphilis





Abstract
We reviewed 20 skin biopsies from the patients of secondary syphilis and analyzed histopathologic features of the skin lesions in 6 cases of macular and in 14 cases of papular lesions, in order to investigate the diagnostic points by the histopathology.
The results were as follows:
1) The histologic features of epidermis in macular lesions were hyperkeratosis in 3 cases (50%) and spongiosis in 3 cases (50%), while in papular lesions were acanthosis in 10 cases (71.4%), spongiosis in 12 cases (85.7%), exocytosis in 12 cases (85.7%), including 5 cases (35.7%) of microabscess.
2) The histologic features of dermis in macular lesions were endothelial proliferation in 4 cases (66.7%), infiltration of inflammatory 4 cells in all cases including 4 cases (66.7%) of plasma cell infiltration and 2 cases(33.3%) of eosinophilic infiltration.
In papular lesions were noted papillary edema in 12 cases (85.7%), endothelial proliferation in 12 cases (85.7%), infiltration of inflammatory cells in all including 10 cases of heavy infiltration 14 cases of plasma cell infiltration and 8 cases of eosinophilic infiltration.
3) Therefore, among the histopathologic features of the secondary syphilis diagnostic points were not identified in macular type but rather diagnostic in papular lesions, showing microabscess, endothelial proliferation and infiltration of plasma cells. Familiarity with these vaiations is most important for proper histologic diagnosis of secondary syphilis.
